How to Create NPS survey form in word/PDF with radio button clickable and with send button to email that form after survey completed

    Hi there,


    I really need your help as in my company we have to send NPS survey email to our client and we need word and PDF file with radio button for saying rate our services from 0 to 10 and client can click on radio button associated with any number. for example -


    1.       Communicating effectively

    0 – Not at all Likely 10 – Extremely Likely
